Housing is a critical social determinant of health and a lack of safe, affordable housing creates significant downstream health care costs. That's why institutions around the country - from Managed Care Organizations to large health systems - have begun to address a range of housing needs in the communities they serve. Their approach and how they received institutional and community buy-in holds important lessons for our own health professionals, housing practioners, and policy makers as we seek to improve health outcomes while using limited resources more effectively. 

On Thursday, December 13 at Thomas Jefferson University Hospitals, PACDC will bring together practitioners from around the country who have implemented successful models to address housing needs to share their findings and provide their insight into how we can replicate these approaches here in Philadelphia and the broader region.

This full day Summit will feature Dr. Kelly Kelleher of Nationwide Children’s Hospital in Columbus, OH and Sister Mary Scullion of Project HOME as keynote speakers, lunch, and a full afternoon of workshops and networking opportunities.
