The Alliance for Health Equity Announces Union Community Care Selected as Core Provider of Healthcare Services at Coatesville's Brandywine Center

Coatesville, PA – January 16, 2025 - The Alliance for Health Equity (AHE) announced today that Union Community Care has been selected as the Federally Qualified Health Center (FQHC) to operate at AHE’s Brandywine Center located at 744 East Lincoln Highway, Coatesville, PA. Beginning July 2025, Union Community Care will start delivering accessible, inclusive, integrated healthcare services to meet the evolving needs of the Greater Coatesville community as part of AHE’s ARPA-funded integrated health strategy.

As the core provider in this model, Union Community Care (Union) will deliver patient-centered, tailored care that integrates primary medical care, maternal and child health, dental care, behavioral health, and social support services.  As a nationally recognized Patient-Centered Medical Home certified for quality and excellence, Union Community Care is a trusted FQHC with a proven track record of delivering its “signature whole health, whole community model of healthcare”.

As the outgoing FQHC, ChesPenn has committed to continuity and collaboration as Union Community Care assumes operations. This warm handoff represents a shared purpose, with ChesPenn Health Services and Union Community Care working together to ensure a seamless transition for Coatesville patients, staff, and community members. Union’s dedicated Care Teams will reach out to ChesPenn patients starting in June, offering personalized support and coordinated care to ensure continuity and trust. The Alliance for Health Equity and Union Community Care are committed to working closely with local stakeholders to ensure the health and wellness of the Coatesville community remain a top priority.
